Web Access to Medical Records
Our clients can have secure access to all of the
records requested from any web-connected computer in the world. Records can be
reviewed, searched, summarized and can be printed from your web browser. No software
purchase is required. The records are obtained from the treatment facilities,
checked for completeness and accuracy, put into chronological order and indexed
by admission. Our imaging department then scans the documents and uploads them
to our website. Your records are accessible by only you and your authorized personnel.
Records digitally stored on CD Rom
We also offer digital images of medical records
on CDs.

at hospitals and doctors' offices since 1990 and has developed a method for communicating
with these custodians on a professional level. We have earned an excellent
reputation that enables us to obtain cooperation from custodians and prompt access
to their records. We are NOT simply a copy company.
Chronological Organization and customized
indexing
Seagrove, Inc. can provide detailed indexing of
medical records for expedited review. We customize and index each set of records
as to content. Many of our clients tell us that the way we index records makes
reviewing them much more productive and saves a significant amount of time.
Records Retrieval Methods
Admissible: A subpoena is issued
(TRCP 176) and served with a deposition on written questions (TRCP 200) to prove
up the records as business records in accordance with TRCE 902(10).
Inadmissible: A subpoena is issued,
but the deposition on written questions does not fulfill the self authentication
requirements set forth by TRCE 902 (10). A separate business records affidavit
is also obtained.
By authorization, with an Affidavit: We deliver
a letter to the custodian of records with an authorization to release them and
an affidavit TRCE 902(10) to prove them up.
Public records, certified copies: Admissibility
is based on TRCE 902 (4), Certified Copies of Public Records.
X-Ray Duplication:
Our lab offers reliable overnight services at
a very reasonable price. Plus, we will pick up and deliver the X-rays at no additional
cost to you.
Oversized Copies (Fetal Monitor Strips):
Our production facility is capable of copying
these files for you so that all the copies are continuous and easy to read.
Process Service:
Subpoenas (to appear
in court or at a deposition): Whether you want us to draft one for you
or pick one up that you have prepared, we serve subpoenas within any Texas court's
jurisdiction.
Citations/TRO´s/Injunctions: The
process servers at Seagrove, Inc. are NOT authorized under Rule 103 to serve
process in State cases.

How
long does it take to get records?
The entire process takes approximately five weeks
including the 20-day notice period (TRCP 200.1). During this time the necessary
research is done to verify the existence and location of records to save you
time and money. In the case that the facility will not release the information
over the phone, we will initiate the necessary paperwork to assure expedient
processing. The remainder of the time is spent corresponding with the custodian
and, once the records are obtained, carefully organizing, numbering, indexing,
copying and binding the records so that they can be delivered to you in a productive
fashion. If you are ever in a bind and need them immediately, there are
instances when they can be obtained in a matter of days.
What
type of records can you obtain?
We can obtain almost every type of record imaginable. Some
of the types of records are medical, billing, x-rays, pathology, personnel and
payroll, Workers Compensation files from employers, claim files from insurance
companies, telephone records (local, long distance and mobile), bank records,
pharmacy records, police records, fire records, driving records, academic records,
ambulance records and business records of any kind.
Can
I specify the way my records are indexed?
Absolutely. We realize that indexing provides
a fast and reliable way to review the records. We try to customize each
set of records as to the content. Since we do not use preprinted indexes, we
will be more than happy to accommodate outside requests with regards to whatever
sequencing you require. If you'd like us to deliver the records in the exact
order in which they were produced, just let us know.
Will
you call me for a fee approval before picking up the records?
Definitely. We realize that you have boundaries
to work within and want to help you avoid any long term complications. It is
fair to say that we have a good grasp of what the industry norm is for pricing.
With the Senate bill guidelines, it is common for an average size hospital chart
to be $125-175. If you have different parameters for a specific case, just let
us know. Keep in mind that the lower this amount is, the more time will pass
before the facility will start copying, thus delaying your records. Seagrove,
Inc. will always attempt to lower exorbitant fees by citing the different rules
that govern pricing.
Can
I subpoena government records?
You can obtain government records with an originally
signed release form by the individual to whom the records pertain. Social Security
Claim records, Wage Information, tax returns from I.R.S. all take 3 – 6
months. Seagrove, Inc. can obtain records from most government agencies
except the Texas Workers Compensation Commission. Any records from the TWCC must
be requested directly by the assigned attorney. Seagrove, Inc. has full
access to any necessary forms needed and can supply you with them upon request.
Do
you copy X-rays?
Yes. In most cases the turnaround time is 24 hours.
However, if you need them sooner, every effort will be made so you can meet your
deadline.
Can
you do on-site copying?
Yes. We have the equipment needed to copy charts
at the witness' office. This is preferred because it ensures legibility and completeness. It
also allows us to copy flow sheets, fetal monitor strips and any other oversized
original that is best viewed in one piece.
Will
you direct bill my client?
Yes, with the appropriate information, we can
direct bill your client. Once we have this information we will direct bill on
all subsequent orders placed. This also applies to co-defendants ordering copies. Just
send us your request with the necessary information. Unfortunately, we
cannot direct bill individuals involved in a family law matter.
Will
you split the taxable cost between all co-defendants?
Yes, but this information needs to be given to
your representative at the time the order is placed. This eliminates any
problems concerning billing when the records are delivered.
Do
you charge a RUSH charge?
A
RUSH charge is only assessed when the records are needed within 48 hours from
the time the order is placed. This is due to the scheduling changes and possible
overtime required to meet your needs.
